We help families understand their needs, narrow down their options, tour trusted communities, prepare for move-in, and feel supported before, during, and after the move.

New Beginnings: Settling-In Support

The move itself is only the beginning. The first few days and weeks in a new community can shape how comfortable, connected, and supported someone feels. We offer thoughtful settling-in support during that vulnerable window, with warm guidance, firm advocacy, and practical help as questions come up.

With nearly a decade of experience in independent living, assisted living, and memory care, along with our experience as a Certified Dementia Practitioner supporting families through dementia and cognitive change, this support package is designed to make those early weeks feel less overwhelming and far more supported.

Has your loved one already made a move but is struggling with the transition? New Beginnings is also available as a standalone service.

What’s Included:

4 to 5 personalized visits during the first 30 days after move-in, used where support is needed most

Support during meaningful moments of adjustment, like joining a meal in the dining room or attending an activity for the first time

Warm introductions to key community team members and guidance navigating how the community works

Help working through early challenges, including apartment setup concerns, unmet expectations, or questions that come up along the way

Ongoing communication, advocacy, and support, including help addressing concerns or requesting care plan updates when needed

In-Home Assessments

Our In-Home Assessment gives a clear view of your loved one’s daily life, safety, and needs. We visit your home, understand your unique situation, and provide personalized guidance to support comfort, independence, and peace of mind.

Ongoing support packages are designed for families who want steady, hands-on guidance over time. From regular check-ins to crisis response and care coordination, these packages provide continued support as needs change.

Foundations Package – starting at $6,000/year

Sometimes you don’t need a monthly package. You need help now. Our on-demand support is for families facing an urgent situation or trying to tackle a one-time need, like getting services set up, sorting through paperwork, or just trying to figure out what to do after an unexpected change.

We’ll start with an immediate consultation and a focused assessment of what’s happening. From there, we’ll create a clear next-step plan and provide additional support as needed, customized to the scope and urgency of your situation.
